​The science behind EWOT

Exercise With Oxygen Therapy is based on Oxygen Multistep Therapy (O2MT), developed by German physicist Prof. Manfred von Ardenne and published in Oxygen Multistep Therapy: Physiological and Technical Foundations (Thieme). The book compiles hundreds of scientific references from his Dresden research institute.

von Ardenne’s central finding was that oxygen delivery to the tissues often declines with age and stress. When capillary walls swell, microcirculation slows. Less oxygen reaches the cells, energy production (ATP) falls, and the problem can reinforce itself. His work described this as a reversible “switching process” in the microcirculation — not a one-way decline.

O2MT combined three elements: improved oxygen utilization, high-concentration oxygen breathing, and physical activity to raise blood flow. One of the shorter protocols — about 15 minutes of exercise while breathing high-flow oxygen — is what most home and clinic EWOT systems are built around today.

What the original research specified:

To match the conditions von Ardenne described for an effective exercise-with-oxygen session, you need:

• High-flow oxygen, commonly 8–10 liters per minute

• High purity, typically about 87–95%

• A mask that actually delivers that oxygen during exercise (not a standard hospital mask, nasal cannula, or headset that dumps oxygen into the room while you exhale)

• Moderate exercise that raises heart rate and circulation for roughly 15 minutes

Lower-flow units (especially 5 LPM and below) were designed for resting patients. They do not meet the high-flow exercise conditions this research was built on.

What EWOT Supports

von Ardenne’s framework focused on restoring oxygen delivery and circulation. In practical terms, EWOT systems built on this model are used to support:

• Microcirculation and tissue oxygen delivery

• Cellular energy production (ATP)

• Recovery and physical stamina

• Circulation to the heart, brain, and extremities

• Overall vitality with age

Results vary. EWOT systems are wellness and training equipment, not a treatment or cure for disease.

Who developed EWOT?
EWOT is based on Oxygen Multistep Therapy (O2MT), developed by German physicist Prof. Manfred von Ardenne. His book Oxygen Multistep Therapy: Physiological and Technical Foundations is the main scientific reference modern EWOT systems follow. The short, high-flow exercise session used at home and in clinics is the version of his work most people now call EWOT.

How is EWOT different from von Ardenne’s original protocol?
von Ardenne described several protocols. The original “standard” course was much longer — often about 18 days with extended oxygen sessions. Two hours a day for nearly three weeks is not practical for most people. He also described a shorter “rapid” method: roughly 15 minutes of exercise while breathing high-concentration oxygen. Home and clinic EWOT systems are built around that shorter exercise session, not the multi-week clinical course.

Why isn’t a 5 LPM concentrator enough for EWOT?
A 5 liter-per-minute concentrator is designed for people who need oxygen at rest. von Ardenne’s exercise protocols call for high flow — commonly 8–10 LPM at about 87–95% purity — because you are moving, breathing faster, and trying to raise oxygen delivery through the microcirculation. At 5 LPM you will not match those conditions. That is why Optimize EWOT packages are built around 10 LPM (and higher) concentrators.

Do I need a prescription to use an EWOT system?
No. The oxygen concentrators in our EWOT packages do not require a prescription. They ship from our U.S. or Canadian warehouse. For U.S. buyers there are no duties or tariffs added. If you have a medical condition, check with your doctor before starting any new exercise program.

Does the original research require a reservoir bag?
No. The key conditions are high-flow, high-purity oxygen during exercise and a mask that actually delivers it. A reservoir bag is one way to present a large volume of oxygen at once. An on-demand mask is the other. We offer both.

On-Demand, Reservoir, or Dual — how to choose

There are two proven ways to deliver high-flow oxygen during exercise. Both can meet the conditions described above: roughly 8–10 LPM, about 87–95% purity, a proper EWOT mask, and about 15 minutes of moderate exercise.

On-Demand EWOT
You turn on the concentrator, put on a specialized On-Demand mask, and start exercising. There is no bag to fill. The mask is built to capture oxygen as it has a small reservoir bag attached that captures the oxygen that would otherwise be lost when you exhale, so you receive the concentrator’s output instead of losing half of it into the room.

Best when you want to start immediately, have limited space, or prefer a simpler setup with no reservoir bag.

Reservoir EWOT
A 1000 liter bag is filled with oxygen from the concentrator before you exercise. You then breathe from that bag through a Reservoir-style mask. During the session you are breathing stored high-purity oxygen, not a mix with room air. A 10 LPM concentrator takes about 90–100 minutes to fill a 1000 L bag. That fill time is normal based on any 10 liter per minute oxygen concentrator.

Best when you want the highest oxygen concentration during the session and you can plan the fill ahead of time. Our reservoir bags do hold the oxygen for up to a week without having to refill if you want to prefill.

Dual: On-Demand and Reservoir in one package
Some days you have time to fill the bag. Some days you want to start now. A Dual package includes the equipment for both methods, so you are not locked into one routine. Optimize is the only company that sells a true Dual system — On-Demand and Reservoir together — instead of forcing you to choose one style and give up the other.

Neither method is a compromise. On-Demand is not “lesser EWOT.” Reservoir is not “inconvenient EWOT.” They are two delivery methods for the same high-flow exercise session. Choose the one that matches how you actually train — or choose Dual and keep both.

What every system still needs:

• A concentrator that can deliver 8–10 LPM at about 87 –95% purity. A 5 LPM unit is for people breathing oxygen at rest, not for EWOT.

• The correct mask for that method. A hospital non-rebreather, nasal cannula, or aviator headset do not work as they will dump oxygen into the room while you exhale.

• Stationary exercise: treadmill, bike, rebounder, or similar. Packages do not include the exercise equipment.
